International Verification

Dear International Customer,

Due to recent international shipping issues we now require all international customers to fill out this form, attach an image of the credit card being charged at Angelic Dreamz, and provide a copy of identification (driver’s license, passport, etc.). Please sign the form you received at the bottom to verify all information is true and email that separately to Sales@AngelicDreamz.com.

Unfortunately due to the recent issues we have been facing we will no longer ship international orders until we receive a signed form with the valid credit card information being used on the order. 

You will only need to do this once for each credit card you would like to keep on file at Angelic Dreamz.
